Enhance Concierge *One-Time Visit* Agreement

This agreement is between Edinger Medical Group and the patient for a one-time visit which will serve as a trial for the Enhance Concierge Medicine Program in Newport Beach (180 Newport Center Dr., Suite 265, Newport Beach, CA 92660).
Concierge/Membership Medicine Benefits
Dr. McConnaughey limits the number of patients enrolled in the Practice in order to provide additional services to his patients. These services which are not provided in a regular medical practice include the following:

Enhance Annual Membership - $4,200/year
- Limited wait time
- Priority appointments (same-day or next-day)
- Prolonged, unhurried office visits
- Dedicated phone numbers to expedite communication and appointment scheduling
  - Office Hours Line: For use Monday-Friday (8:30am-5pm, holidays may differ)
  - After-Hours Line (emergencies only): For use Monday-Friday nights (5pm-8:30am), plus weekends & holidays
- In-office lab draws for convenient bloodwork
- Access to IV services
Acknowledgment

By signing this agreement, the patient agrees to pay a $600 fee associated with the one-time visit option in the Enhance Concierge Medicine Program. Included in the cost of this visit is one 60-minute physical with Douglas McConnaughey, M.D. This fee is non-refundable and will be applied toward the annual fee if the patient decides to complete the membership enrollment process. The patient will have 48 hours after the visit is complete to apply this $600 fee towards an annual membership option.
